Who we are

We are Meadowfield, an independent community pharmacy registered with the General Pharmaceutical Council (premises 1234567). Our superintendent pharmacist is Imran Patel.

Information we collect

To dispense medicines and deliver clinical services safely, we collect identifying information (name, date of birth, NHS number), contact details (address, phone, email), and relevant medical information (prescriptions, allergies, consultation notes).

How we use your information

We use your information solely to provide you with safe pharmacy care, to communicate with you about your medicines and appointments, and to fulfil our legal obligations under NHS and pharmacy regulations.

Who we share it with

With your consent we share clinical notes with your GP. We never sell your data. Anonymised, aggregated data may be shared with NHS England for service improvement.

Your rights
